Jack is preceded in death by wife, Mary; grandson, Keenan Cornwell; son-in-law, Tim McCrady; daughter-in-law, Donna Cornwell.

Survives include children: Polly (Roger) Hines, Kevin (Chris) Cornwell, Rebekah (Bob) Moyle, Michele McCrady (fiancé Kevin Morrison), and Mitch Cornwell; grandchildren: Chris (Laura) Hines, Tiffany (Brandon) Jacoba, Fjeran Moyle (fiancé Drew Olsen), Hudson (Dana) Moyle, Savannah Moyle, Hannah Cornwell, Alyssa (Tyler) Speck, Rand McCrady, Dallen (Kaitlyn) Cornwell, and Shane Cornwell; six great-grandchildren and other Minnesota relatives.

Visitation: Wednesday, July 11th at 4 P.M. at the West Center Chapel, followed by Vigil Servie at 6:30 P.M.

Services: Thursday, July 12th at 10 A.M. at St. Joan of Arc Catholic Church, 3122 S. 74th St.

Interment: Calvary Cemetery.

Memorials: Suggested to the American Lung Association.
